Individual Support in the community
We can provide one-on- one support to assist you to access and participate in the community seven days a week.
These supports can be scheduled as one- off bookings or as an on-going.
Some of the activities we assist with:
- Grocery Shopping
- Medical appointments
- Accessing the library
- Going to the bank or post office
- Visiting friends and family
- Going to a café or restaurant
- Hair, nail and beauty appointments
- Attending community events
- Community volunteering
- Wheelchair access vehicle
Assistance with Daily Living
Our daily living supports are flexible and tailored to your own needs, goals and outcomes,
we offer invidualised supports in your home.
You might want:
- Support getting ready for your day (personal care)
- Support with household chores
- Assistance with meal preparation
- in-home respite
Terms and Conditions Include:
There are limited places available for these activities,
so please RSVP with at least 2 weeks prior to the activity to secure your spot.
Activities will be cancelled if minimum numbers are not met.
You will also require an initial assessment and a service agreement prior to the group activity.NDIS Supported hours required: 8hrs two Saturdays a month.4hrs two Saturdays a month.6hrs every Sunday.
Concert tickets, sporting events, leisure activities and meals are not covered under the NDIS hours.All programs are small group activities. Meeting point will vary locations across Western Sydney.NOTE: Temporary Transformation Payment (TTP) supports and codes will be used on all new service agreements.
We recommend participants contact their support coordinator, local area coordinator, plan manager or case worker, for advice about linking prospective NDIS funding to their personal goals.
